Community Care Mapping
What it is
Community care mapping is an ideal method for convening providers from different organizations to identify and explore shared challenges and opportunities. Different personas spark discussions on topics such as mental health, substance use, disability, parenting, and more.
Who it’s for
Community care mapping is suitable for groups who may not always work together but have a shared interest in their community and its people. While originally designed to look at integration between healthcare and other services, it can be adapted to any service sector in a community or region.

Team Mapping
What it is
Team mapping is a method for improving team function and effectiveness by exploring and refining roles, tasks, scope of practice, and workflows. Patient personas prompt discussions about realistic scenarios encountered in practice. Use it as a tool for brainstorming, onboarding, or team-building.
Who it’s for
Team mapping is suitable for teams at any stage of development, whether you’re a newly formed team still adding members or an established team interested in continuous improvement. While built for team-based care in primary care clinics, it can be adapted to work for teams and clients in any kind of organization.
